Request help with adding Hebrew name of POI in Ukraine (memorial)

The Holocaust memorial plaque in the village of Solotvyno, Ukraine (coordinates 47.9466948, 23.8752858) has a text in both Ukrainian and Hebrew:

I don’t read Hebrew, nor do I have a Hebrew keyboard. So, I thought this would be a good place to ask for help. Could someone please add the correct name:he tag to this node based on the text in the photo?

I can read Hebrew but I’m not sure what should be the name:he tag for this place.
The Hebrew text is:
“על הגל הזה רעדה המצבה הזו”
יהודים - קרבנות השואה
נזכור ולא נשכח
My, probably not so good, translation to English goes as follows:
“On this wave this gravestone shook”
Jews - Holocaust victims
We will remember and not forget

Small correction to Harel’s transcription of the first sentence:
עֵד הַגַּל הַזֶּה וְעֵדָה הַמַּצֵּבָה

This is from Genesis 31:52.
The English translation (from biblegateway.com): “This heap is a witness, and this pillar is a witness”
