Build maps form cannot be found - or I'm missing something obvious

Apologies if I'm doing something stupid but I am new to this.

I'm trying to download a map of Slovenia from http://garmin.openstreetmap.nl/

I've tried selecting the country of interest (Slovenia) and nothing happens.

I've tried 'drag selecting' the tiles of interest and nothing happens.

The page says to " REQUEST YOUR MAP OR DOWNLOAD IT DIRECTLY:"  but
how does one make the request?

In the Wiki pages there is this:

"Once you've selected the map tiles, filled out the form and clicked
the 'Build maps' button, your request is queued on the server. The
server processes each request in four steps: "

I can't see/find a form.

Am I missing something really obvious?

Thanks for the report. It was caused by a failed map update and has been fixed yesterday.
